Your primary responsibilities will include:
- Conducting thorough physical assessments and reviewing medical histories to evaluate patients' conditions.
- Creating individualized treatment plans tailored to each patient's specific needs and rehabilitation goals.
- Implementing therapeutic exercises and training programs to enhance mobility and functionality.
- Educating patients and their families about injury prevention, wellness, and recovery processes.
- Monitoring and documenting progress towards rehabilitation objectives, adjusting treatments as necessary.
- Collaborating with other healthcare professionals to ensure comprehensive care.
- Maintaining precise patient records and documentation in accordance with regulatory standards.
Qualifications:
- Masters degree in Physical Therapy.
- A minimum of 1 year of experience in home health as a physical therapist.
- Valid state licensure to practice as a physical therapist, adhering to professional standards and practices.
- Current CPR certification required during clinical employment.
- Valid driver's license with the ability to travel to various client locations.
